When people come to me with upper back pain, shoulder tension, or even radiating arm symptoms, one of the first places I check is the chest. It surprises many clients to learn that tight pectoral muscles are often the primary driver of their pain — not the upper back muscles they've been stretching and foam-rolling without relief. Chest tightness is one of the most overlooked causes of upper body discomfort, and it's epidemic in our screen-forward world.
Here's what happens: when you spend hours with your arms in front of you — typing, driving, cooking, scrolling your phone — the pectoral muscles shorten. As they shorten, they pull your shoulders forward into that rounded posture you might recognize in the mirror. This forward pull forces your upper back muscles to constantly overwork trying to hold you upright, creating that burning, aching pain between your shoulder blades. The bra-line pain that many women experience is often rooted in this same imbalance. And when the chest wall tightens enough, it can compress the nerves and blood vessels that travel to your arms, creating thoracic outlet-type symptoms like tingling, numbness, or radiating arm pain.
Treating the front of the body is just as important as treating the back, and it's something I address in nearly every session. Releasing the pectoralis major and minor, the anterior deltoid, and the subclavius opens up the chest, takes pressure off the compressed structures underneath, and allows the shoulders to settle back into a more natural position. Once the front releases, the upper back muscles can finally stop compensating and start recovering.
